About Varicose Veins
Varicose veins are a genetic illness that has been around since the dawn of ages. It is made worse with pregnancy, weight lifting and standing on your feet all day long. Therapy is usually divided into two modalities, Cosmetic Sclerotherapy and Medical Therapy. The Medical Therapy applies when there are either Varicose veins (veins that are greater than 4 millimeters and refluxing) and/or Perforators (veins that connect the superficial veins with the deep system).
Cosmetic Sclerotherapy has basically only two options. If you have spider veins of the lower extremity then the only really effective treatment is injection Sclerotherapy. There are still some physicians who are unsuccessfully trying to laser the leg veins. It does not work and usually makes the veins worse. The exception is the face where injections can be dangerous. In this area we recommend either IPL or Laser therapy.
The Medical veins are usually treated with Lasers or Radio-frequency catheters. The results are essentially the same. The incompetent Perforators can be treated with either lasers, radio-frequency catheters or foam injections.
If the veins are too large and tortuous (curvy) then one has to do a procedure called Ambulatory Phlebectomy. This is where under local anesthesia we make tiny (almost invisible) cuts and take the vein out physically. It is time consuming but the results are usually spectacular!
There has been a resurgence in interest in Varicose veins because of the simplicity of treatment today, the high success rate when performed properly, the ability to return to work the next day, and the great training that has occurred with the American College of Phlebology.
Unfortunately there are all too many Physicians who are not properly trained and performing sclerotherapy. My biggest pet peeve is when a patient comes in to see me, has not had a screening doppler exam by the physician and there is underlying reflux. I tell all of my patients that if someone just looks at your veins and wants to do injections without a doppler exam leave the office immediately.
